Haliburton GAIN Clinic:

Provides Geriatric assessments to frail seniors living at home whose health, dignity, and independence are at risk. Seniors will be assessed and treated at home or in the clinic as needed. (does not assess Long Term Care residents)

Service includes:

• Complete and comprehensive assessment
• Develop a personalized care plan
• Provide follow-up in the home
  

The objective of the GAIN team is to work with seniors and their circle of care including caregiver(s), primary care practitioner and other service providers to help;

• Improve senior home safety and their ability to live independently at home
• Improve the quality of life experienced by the senior, their caregivers and family while living in their home
• Reduce the frequency of preventable visits to local Emergency Departments and hospital admissions/re-admissions
• Shorten the length of hospital stays.
• Delay admission to Long-Term Care Homes as long as safely possible

The GAIN team includes;

Nurse practitioner * Occupational Therapist * Social Worker * Registered Dietitian * Registered Practical Nurse (certified in behavioral support) * Home support worker * Administrative assistant * Geriatrician or physician who specializes in geriatrics (available for consultations when required).

• Are living at home or in a Retirement Home (does not serve Long Term Care residence)
• Have multiple complex health problems or medication issues
• Have experienced a functional decline or are at risk for falling and other safety concerns
• Have experienced a cognitive decline, psychosocial issues, and/or have difficulty coping

• Be referred by any healthcare practitioner in the community, emergency department, self or caregiver or upon discharge from the hospital.
• Live at home or in a retirement home in Haliburton County
• Have a scheduled appointment - walk-ins are NOT accepted
