Description (Service)

Family-centered care program designed to assist those diagnosed with a terminal illness or a life limiting illness (chronic disease)

Support is provided to individuals and families in home, hospital or nursing home
Volunteers provide companionship, assistance with light household duties, and simple comfort measures
Bereavement support and grief programs include one-on-one support, monthly caregiver-support groups, and ongoing social support groups. Any individual who has suffered a significant loss is welcome to bereavement support
Respite service provided for family members who are caring for a love one. Advanced care planning workshops and presentations available to groups
A visiting hospice - no beds provided
