Provides two transportation programs for residents unable to use public transit due to a physical, cognitive, visual or sensory disability - Transportation Linking Communities (TLC)
- Midland Accessible Transportation (AT)
Services offered include transportation to: - COVID-19 vaccine appointments
- regular medical appointments
- specialized health services appointments such as dialysis or chemotherapy-local and out of town
- shopping
- social events

The Medical Assessment Form completed by a Health Care Professional along with the Transportation Application Form must be submitted to Community Reach to establish eligibility. |
