The In-Home Caregiver Relief program is intended for those who have Alzheimer's or related dementia. 18 year(s) and up Does not provide transportation to individuals in Long term care homes (that is provided directly by the LTC home / Home and Community Care Support Services). |
Application | Transportation to Dialysis appointment must be arranged through the Social Workers at the Dialysis Unit. Please call for further information about the Transportation program and any other services. |

Volunteer Description | CHATS is seeking volunteers for our Adult Day Program, Wellness/Lunch out Programs, Transportation program, Meals On Wheels and Telephone Reassurance Program. Flexible times available, generous mileage reimbursement. |
