Description (Service)

Provides supportive, affordable housing to those who are living with a serious mental illness, which also affects their day-to-day living.

  • supports individuals to achieve and maintain physical, mental health, and well-being, as well as foster independence and enhance participation and integration into the community of their choice

  • person-centred,flexible service, reflecting the individual and their changing needs

  • promotes active participation in their own planning and ensuring the opportunity to makeindividual choices

Hours Office: Mon-Fri 9 am- 5pm, 24 hour supported living
Areas Served Simcoe County
Eligibility Ages 16 years and older with a serious diagnosed mental illness looking to live in a congregate residential living setting, with ample support.

Must be:
  • psychiatrically stable with a demonstrated adherence to treatment plans
  • assessed to be capable of living within community setting if appropriate supports are available
  • able to reside within a supported congregate living environment with multiple roommates of varying ages and abilities
  • medically stable, and if experiencing ongoing or complex physical health issues, appropriate home-based health supports are or will be in place (e.g., Home & Community Care, Personal Support Workers, Private Nursing, etc.)
  • able to take direction from others in an emergency situation
  • able to navigate stairs independently
  • able to complete ADLs independently with prompting (i.e., eating, bathing, personal hygiene, toileting, dressing)
  • able to provide financial documentation proving source(s) of income
Application Complete the online referral form and email it to chointake@cmhastarttalking.ca. If assistance with completing the referral form is required, call 705-726-5033 ext. 438 for support. If after the initial screening the criteria laid out above has been met, an intake appointment will be scheduled, where further information may be gathered.

If the criteria for the CHO program continues to be met, the operator of your home of choice, or available home, will be in contact to schedule a tour and finalize the application process.
