Description (Service)

Provide information, education and support for persons living with dementia and their care partners. The following programs and services are offered:

  • Navigation (connects individuals newly diagnosed and/or who have not accessed other support / community services)
  • Supportive Counselling
  • Care Partner Support Groups
  • Education programs for persons living with dementia, their care givers, professionals and the general public
  • Social, Recreational and Exercise programs
  • Resource Library & Information packages
  • Activity Kits

Member of the North Simcoe Muskoka Community Support Services Collaborative and the North Simcoe Muskoka Dementia Network.

Hours Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm

Eligibility People with Alzheimer's disease and other dementias and care partners.
Application
  • Call, email or drop-in to access general information and/or to start the process of accessing the services
  • Appointments are required to speak with a Family Support Coordinator
  • Members of the public can self-refer and family members can refer loved ones by contacting the office
  • Community partners and Family Physicians can refer clients by using the First Link Referral form
