Description and Service Details

Description (Service) Key Services and Support Coordinators work in partnership with families, assisting caregivers with FASD education and in navigating relevant resources and linkages to appropriate services
Support includes:
  • Developing a flexible support plan, monitoring and updating plan as needed
  • Supporting connections to diagnostic supports
  • Building capacity and identifying strategies with family, services providers, and educators
  • If appropriate, connecting to Coordinated Service Planning Team
  • Helping to create transition plans to adult services
  • Identifying gaps and opportunities for improving supports and awareness for FASD by supporting cross-sectoral planning tables
  • Engaging with different cultural groups
  • Facilitating parent-to-parent connections

Eligibility Children and Youth 18 years and under with FASD (or 21 if still in secondary school)

  • Parents and professionals may call a services navigator at the Children's Treatment Network ACCESS: 1-866-377-0286 for more information about coordination services under the program.
  • Individuals who are involved with a service agency but do not yet have a diagnosis of FASD, assessment can be arranged by speaking with their existing service provider
  • Individuals who are not involved with a service agency should speak with a doctor, community health nurse or community health centre to ask about the FASD assessment process
