Description (Service)

Housing and a unique mix of customized support to seniors and adults who are struggling with mental and physical health challenges, substance use, poverty, behavioral challenges, and homelessness.

Services Include:

  • Transition from Hospital (ALC) - specialized housing and supports to help adults and seniors safely and successfully return to living in the community.
  • Permanent Housing - helps clients gain housing based on their needs and remain independent in homes of their own, for as long as possible.
  • 24/7 Support - staff provide around the clock crisis management, intervention, and prevention for clients who are unable to live independently due to serious mental health issues or other complex challenges. For adults and seniors who require it, we provide full meal service and support with activities of daily living. For youth, we provide support and training with meal preparation.
  • Assisted Living - assisted living services for high-risk seniors enable clients to enjoy their own space in an apartment or group home environment. Helping people coordinate services from other social service and healthcare providers, connect with doctors, nurses, and other healthcare professionals, manage their medications, and organize escorts to appointments as required.
  • Transitional Age Youth Housing Services - Shared supportive housing is available for young people with mental illness and addiction challenges, usually between the ages of 16 and 26 and in some cases up to 29. These young people receive specialized, recovery-focused care while they are in school or work
  • On-Site Residential Support - For clients with less demanding challenges, we offer medium-support and low-support housing through LOFT-owned-and-operated apartment buildings and shared homes. Ideal for clients who prefer an independent, or nearly independent, living environment. We also offer ongoing support to clients living in apartments of their own.
