Description	Connects individuals with a wide range of services including case management, information and referral, professional health services and supportive care services * see list below for the many areas of healthcare, home health care, and health service providers NSM CCAC works with in your community to connect you to the best care for your situation:  In Home Services  Nursing  Physiotherapy  Occupational Therapy  Speech and Language Pathology   Social Work  Nutritional Services  Respiratory Therapy   Personal Support  Medical Supplies  Equipment Rentals  Adult Day Services  Residential Hospice  Long Term Care Admissions    Respiratory Therapy component of CCAC services is provided to individuals who use ventilators, have artificial airways or are receiving home oxygen services under the Assistive Devices Program    Member of the North Simcoe Muskoka Community Support Services Coalition

Eligibility	Children * adults * seniors * residents of area served    In order to receive services from the CCAC, client must have   * a valid Ontario health card   * a need that cannot be met on an outpatient basis   * a medical condition that can be adequately treated in the home   * a need for at least one health care professional or personal support service
