| Description | Services for patients with more than one therapeutic need following an illness or injury that has occurred within the last 3-4 months, usually following stroke, acquired brain injury, spinal cord injury or amputation * most patients attend two times per week for approximately 8-12 weeks * program is available five mornings per week * assessment and treatment is provided by nursing, occupational therapy, physiotherapy, recreation therapy, rehabilitation assistant, social work and speech language pathology
